Help for PDF Files

Some application forms are available in Adobe Acrobat PDF format. To be able to view or print these applications, it is necessary to download and install the Adobe Acrobat Reader. After you install Adobe Acrobat Reader, you will be able to open, view and print these forms.

Once the document is open you may print it using your printer so you can fill it out. Complete the application, sign and date it, then mail it to us with the application fee. Be sure to have your high school or college/university academic records (transcripts) mailed to us, too!
